Sacrament Sponsor Letter

- If you are a registered parishioner of St. Mary of the Assumption and you have been asked to act as a Godparent or as a Confirmation or OCIA sponsor for a friend or family member, please use the form below to request a Letter of Eligibility. The purpose of the letter is to confirm that you are a practicing Catholic and have received the three Sacraments of Initiation: Baptism, Eucharist, Confirmation. We will send the letter directly to the church where the sacrament will be preformed.
